Night Shift Differential: $4.25
PURPOSE OF POSITION: Provide professional nursing care within established Oregon Nursing Practice Act; and established Nursing Policies and Procedures. Coordinate care planning with other disciplines. Delegate duties to other nursing staff as appropriate. Contribute to daily patient care management by modeling professional practice.
ESSENTIAL JOB FUNCTIONS:
Provide direct patient care, evaluate patient changes/outcomes, consult with other health care providers as necessary and adjust nursing care processes as indicated to ensure optimal patient care.
Assess and re-assess patient status and intervene appropriately within licensing authority. Adjust plan of care in response to changes in patient needs, inform physician and document findings.
Maintain accurate and current nursing documentation including patient histories, medications, conditions, plans, treatments, responses and assessment of changes.
Treat patients and their families with respect and dignity. Identify and address psychosocial, cultural, ethnic, age-appropriate, and religious/spiritual needs of patients and family. Act as a liaison between the hospital, patients, physicians and other healthcare providers.
Interact professionally with patient and family and involving them in the formation of the plan of care and document that involvement. Formulate a teaching plan based on identified learning needs and evaluate effectiveness of learning. Include family in teaching, as appropriate.
Reconcile medications upon admission, when level of care changes and upon discharge. Prepare and administer medications. Prepare, hang, monitor and discontinue IV fluids; may delegate to Licensed Practical Nurse with appropriate certification, however maintain authority to evaluate patient responses and to ensure appropriate intervention. Consistently assess pain, medicate as authorized and document.
Evaluate patient infectious process or disease. Evaluate patient areas for potential sources of pathogenic transmissions. Evaluate need for isolation precautions as appropriate to the disease process, and initiate precautions and/or isolation as needed. Apply infection control policies and procedures.
Provide patient reports at the patient’s bedside (using SBAR format) when a change in primary care nursing occurs and to oncoming shifts. Solicit and document patient’s involvement in his own plan of care.
Be knowledgeable of and maintain established departmental policies and procedures, objectives, performance improvement program, safety, environmental and infection control standards.
JOB QUALIFICATION REQUIREMENTS:
MANDATORY REQUIREMENTS: Thorough knowledge of professional nursing principles and practices and license limitations. Knowledge of mathematics to include basic algebra and geometry, and computer and keyboard operations. Ability to read, analyze and interpret professional journals, technical procedures and governmental regulations. Ability to write medical reports and procedures. Graduate of an accredited school of nursing, or any satisfactory combination of experience and training which demonstrates the knowledge, skills and abilities to perform the above duties.
SPECIAL REQUIREMENTS/LICENSES: Possession of a current Registered Nurse license within the State of Oregon, or ability to acquire by the time of appointment. Must possess; CPR/BLS, ACLS, within one year; NRP, ENCP/PALS and TNCC certification as soon as available per schedule of nurse educator but within the first two years of employment.
DESIRABLE REQUIREMENTS: At least one year (1) of nursing experience. Possession of ACLS, TNCC at time of appointment. Possession of PALS at time of appointment or within one (1) year of appointment. Previous nursing experience in a Rural Critical Access hospital.
